With substantial expansion of use of personal information as society becomes more and more interconnected by telecommunication services, Cosmo Enterprise Co., Ltd. (hereinafter, “Cosmo Enterprise”) takes necessary and appropriate measures for proper handling and security management of personal information based on the principles of the Act on the Protection of Personal Information, which stipulates that personal information must be handled carefully under the philosophy of respecting the personalities of individuals.
In order to manage personal information securely, Cosmo Enterprise creates a management structure and works to improve that.

1. Person responsible

Cosmo Enterprise assigns a person responsible for protection of personal information and appropriately handles and securely manages personal information.

2. Acquisition of personal information

Cosmo Enterprise acquires by legal and fair means personal information within the scope necessary for business.

3. Purposes of utilization of personal information

Cosmo Enterprise utilizes personal information acquired within the scope necessary for the following business and will not utilize it for purposes other than those. Any change to the purposes of utilization of personal information shall be within a scope recognized as having reasonable relation to the purposes of utilization before the change, and purposes of utilization after the change shall be notified to customers or publicly announced.
Purposes of utilization of personal information handled by Cosmo Enterprise as follows.

• Management of conclusion and execution of contracts between customers and Cosmo Enterprise and other transactions in the performance of Cosmo Enterprise’s business
• Contact between customers and Cosmo Enterprise and notices and greetings based on accepted social usages
• Market surveys, customer behavior analysis, etc. related to Cosmo Enterprise’s business
• Exchange of information and contact between concerned persons of industries and organizations related to Cosmo Enterprise’s business
• Replying to customer inquiries, etc. and improving Cosmo Enterprise’s business activities taking into account opinions, requests, and grievances
• Delivery by post or delivery service of products ordered by customers and responses pertaining to those
  

4. Provision of personal information to third parties.

Cosmo Enterprise shall not provide personal information to third parties without obtaining the customer's approval. However, this does not apply in the following situations.

• Cases authorized by law
• Contact between customers and Cosmo Enterprise and notices and greetings based on accepted social usages
• Commissioning within the scope necessary for performing Cosmo Enterprise business
  (In this case, necessary and appropriate oversight of the party commissioned shall be done in order to accomplish security management of entrusted personal data.)
• Joint use with Cosmo Enterprise group companies

5. Joint use with group companies

Cosmo Enterprise group companies (refer to our company profile “About Us” page) may jointly use amongst the companies personal information related to customers as follows in order to provide guidance to customers on products and services handled and to promote sales. Information that is used jointly includes names, addresses, telephone numbers (including mobile phone and fax), e-mail addresses, company/organization names, department names, titles, workplace addresses, and other contact-related information as well as sex, date of birth, transaction history, and questionnaire information. Cosmo Enterprise bears responsibility for management of the jointly used customer personal information in question, and personal information is managed strictly at group companies at a level equivalent to that of Cosmo Enterprise.

6. Management of personal information

Cosmo Enterprise makes an effort to maintain personal information with accurate and up-to-date content within the scope necessary for achieving the purposes of utilization. The company takes necessary and appropriate measures to prevent leakage, loss, or damage to personal information and for other security management. And it conducts necessary and appropriate supervision of employees in order to handle personal information properly.

Basic policy related to proper handling of specific personal information

Cosmo Enterprise stipulates the following basic policy in order to secure proper handling of specific personal information as an organization.

2. Compliance with related laws, guidelines, etc.

Cosmo Enterprise complies with the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures (Act No. 27 of 2013), Act on the Protection of Personal Information (Act No. 57 of 2003), and “Guidelines for proper handling of Specific Personal Information (for private entities)”, and it properly handles specific personal information.

3. Use of individual numbers

（1）Cosmo Enterprise uses individual numbers of clients (individual number as stipulated in the Act on the Use of Numbers to Identify a Specific Individual in the Administrative Procedure (hereinafter, ”individual number“)) for the following purposes.

• Duties for preparation of payment records for remuneration, fees, etc.

（2）Cosmo Enterprise uses individual numbers of employees and their dependents for the following purposes.

• Duties for preparation of employment income/retirement income withholding records
• Duties related to local taxes
• Notification, application, claims, and certificate preparation duties based on the Employment Insurance Act
• Notification, application, and claims duties based on the Health Insurance Act and Employees' Pension Insurance Act
• Notification, application, claims, and certificate preparation duties based on the Industrial Accident Compensation Insurance Act
• Duties related to the National Pension Act category 3 insured persons system
• Duties for submission of reports, notices, and applications related to property accumulation savings/property accumulation pension savings
• Duties for preparation of Japan Airport Terminal employee stock ownership scheme member payment records
• Duties related to the Inheritance Tax Act
• Duties related to payment of benefits or lump-sum payments defined as pension according to the Defined-Benefit Corporate Pension Act

4. Items related to security control measures

In security control measures for specific personal information, Cosmo Enterprise formulates rules for handling of specific personal information and takes organizational security control measures, human security control measures, physical security control measures, and technical security control measures according to Guidelines for proper handling of Specific Personal Information (for private entities).

5. Continuous improvement

Cosmo Enterprise strives to continuously improve handling of specific personal information.
